| The Washington Square Health Foundation recognizes
that the Chicagoland area may receive a large number of displaced
persons due to the Hurricane Katrina disaster. This may result in
an increased need for funding for health care organizations in the
Chicagoland area.
The Washington Square Health Foundation will entertain “Capacity
Grant” requests from Chicagoland health care facilities that
are receiving significant numbers of Katrina disaster victims. The
Foundation believes that this will be the best use of its resources
and consistent with the Foundation’s mission of promoting
and maintaining access to adequate health care for all people in
the Chicagoland area.
The “Hurricane Katrina Disaster Capacity Health Care Grant
Guidelines” are as follows:
A. Capacity Health Care Grants may be applied for in FY 2005-06
for up to $35,000, for one year only. The first application deadline
is December 1, 2005.
B. Applicants for Capacity Grants must document that they will
serve Katrina disaster victims who will represent an additional
10% of the average of the applicant’s unduplicated clients\patients
served during the last three years. These unduplicated clients\patients
should be expected to generate at least two visits per client\patient
in FY 2005-06.
C. Funds may be applied for and used for the following:
- Additional professional, licensed or clerical personnel to
serve the Katrina disaster victims\clients\patients. All clerical
positions under the Capacity Health Care Grants must be filled
by Katrina disaster victims;
- Equipment required to serve the additional Katrina disaster
victims\ clients\patients; and
- Materials and supplies required to serve the additional Katrina
disaster victims\ clients\patients.
D. All other guidelines, grant
application requirements and procedures, as found on the Washington
