Use the following questions to guide you through the preparation
of a project brief. The brief should be no more than 10 pages and
all questions must be answered. Use the word processing application
and format of your choice and attach the brief to the General Application
form when submitting the application.
Objectives are indicators of achievement that are measurable
in some way. All objectives should include an indicator statement,
a target population, a time frame, and the amount of change expected
in the indicator. For example, the number of eye surgeries (indicator)
among older adults (target population) will increase by
50% (amount of change) in one year (time frame).
Outcome and Process objectives are defined below:
OUTCOME OBJECTIVES: Direct results of the program that
are stated in terms of performance and not effort. They should
described changes in knowledge, attitude, behavior, skill, community
capacity, health status or health care utilazation.
PROCESS OBJECTIVE: Activities which must be performed
in order to accomplish the outcome objectives.
SAMPLE 1:
Goal: To improve the ability of low-income famalies
to manage their asthma.
Objectives:
Outcome
The average number of days of school absence will decrease
by 30% among program participants during the first year
of the program.
and/or
Average number of emergency room visits for asthma among
program participants will decrease by 50% during the first
year of the program.
and/or
Ninety percent of parents and children seen in the asthma
clinic will be able to demonstrate correct use of the peak
flow meter.
Process
Clinicians will document patient education provided to
asthma patients/families during their visits 80% of the
time.
and/or
Patient surveys will be administered every six months to
10% of the clinic population.
SAMPLE 2:
Goal: To reduce the use of emergency department for
non-emergency conditions.
Objectives:
Outcome
The number of patient visits to the ER for non-emergency
conditions will decrease by 20% during the first year of
the program.
and/or
At least 75% of follow-up appoinments in primary care clinics
for non-emergent emergency department visits will be kept
during the first year of the program.
and/or
At least 80% of patients seen in pediatric, primary care
and internal medicine clinics during the firest six months
of the program will be able to describe the Ask-A-Nurse
program and options for after-hours care.
Process
Utilization of the Ask-A-Nurse Service will increase by
30% during the first six months of the program.

The pediatric service will ad 10 additional hours per week
of late afternoon, evening and weekend clinic time during
the first year of the program.
